Certified Home Health Aid (CHHA)

  • Location: New Jersey

  • Part-Time / Full-Time / Per diem

  • Position Overview: The Certified Home Health Aide (CHHA) provides hands-on personal care and supportive services that enable clients to remain safe, comfortable, and independent in their homes. Under the direction and supervision of a Registered Nurse, the CHHA assists with daily activities, promotes dignity, and offers compassionate companionship.

Key Responsibilities

1. Personal Care Assistance

  • Assist clients with bathing, dressing, grooming, and toileting.
  • Support mobility needs, including transfers, ambulation, and safe positioning.
  • Provide skin care, oral hygiene, and basic grooming.
  • Assist with continence care and use of assistive devices.

2. Activities of Daily Living (ADLs) & Home Support

  • Help with meal preparation according to dietary needs and preferences.
  • Assist with light housekeeping related to the client’s care (e.g., linens, dishes, tidying client areas).
  • Support feeding as needed and ensure client hydration and nutrition.
  • Provide reminders for medications (no administration unless allowed by state regulations).

3. Companionship & Emotional Support

  • Offer warm, respectful companionship and conversation
  • Encourage engaging activities to promote emotional well-being.
  • Observe and report changes in client mood or behavior.

4. Monitoring & Reporting

  • Observe and report any changes in client condition, safety concerns, or health issues to the supervising Registered Nurse.
  • Document care provided in accordance with agency policies.
  • Follow the established plan of care and agency procedures at all times.

5. Safety & Infection Control

  • Maintain a safe environment for clients by identifying hazards and taking corrective action when appropriate.
  • Follow infection control procedures including proper hygiene and use of personal protective equipment (PPE).
  • Assist clients in emergency situations following agency guidelines.

Qualifications

  • Active New Jersey Certified Home Health Aide (CHHA) license in good standing.
  • High school diploma or equivalent preferred.
  • Ability to follow care plans and written/verbal instructions.
  • Reliable transportation to travel to the client’s home.Valid NJ driver’s license and automobile insurance.
  • Current CPR certification.
  • Successful completion of background checks and health screenings.

Registered Nurse (RN)

  • Location: New Jersey

  • Part-Time / Full-Time / Per diem

  • Position Overview: The Registered Nurse (RN) plays a vital role in delivering compassionate, high-quality care to clients in the comfort of their homes. The RN is responsible for performing skilled assessments, creating individualized care plans, supervising caregivers, and promoting the highest level of safety, dignity, and well-being for every client.

Key Responsibilities

1. Client Assessment & Care Planning

  • Conduct comprehensive initial nursing assessments for new patients.
  • Develop individualized plans of care based on medical needs, functional abilities, and client/family preferences.
  • Update care plans regularly and as needed based on changes in condition.

2. Skilled Nursing Services

  • Provide nursing interventions such as wound care, medication management, and other skilled procedures permitted under state regulations.
  • Monitor vital signs, symptoms, and treatment responses.
  • Communicate changes in condition promptly to physicians, caregivers, and family members.

3. Supervision of Caregivers

  • Supervise Home Health Aides (CHHAs),  and Personal Care Aides (PCAs) in accordance with state requirements.
  • Conduct field visits to observe caregiver performance and ensure compliance with care plans.
  • Provide guidance, training, and support to caregiver staff as needed.

4. Documentation & Compliance

  • Maintain accurate, timely, and thorough documentation in accordance with agency policies and state/federal regulations.
  • Ensure all nursing notes, assessments, and care plan updates meet regulatory and accreditation standards.
  • Participate in quality assurance and performance improvement activities.

5. Communication & Coordination

  • Collaborate with physicians, therapists, caregivers, and family members to ensure coordinated and effective care.
  • Provide education to patients and families on disease processes, medications, and care techniques.
  • Support smooth transitions between hospitals, rehabilitation centers, and home.

6. Safety & Infection Control

  • Adhere to all safety, infection control, and emergency procedures.
  • Identify risks in the home environment and recommend modifications to improve safety.
  • Report incidents according to agency protocol.

Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the state.
  • Minimum of 1 year home care nursing experience.
  • Strong assessment, critical thinking, and clinical skills.
  • Excellent communication, documentation, and interpersonal abilities.
  • Ability to work independently and manage multiple patients.
  • Licensed driver with a valid NJ driver’s license and valid automobile insurance.
  • CPR/BLS certification required.
  • Successful completion of background check and health screenings.

Companion Caregiver
